🏠 Transitional Housing (Humble Reality) – Pre-Qualifications

Crisis JK Solutions partners with Humble Reality to offer transitional housing support. To qualify for referral, you must meet the following:

  • ✅ Complete a full Crisis JK intake application
  • ✅ Be actively experiencing housing insecurity or eviction
  • ✅ Be willing to transition within the 1st–15th of the month
  • ✅ Be able to pay at least 50% of housing and utility costs
  • ✅ Participate in follow-ups with a Dedicated Case Agent
  • ✅ For utility support, use Rhythm Electric and obtain a pledge code
  • ✅ Must not be seeking permanent placement (12-month limit)

  • Eviction Notice

  1. Proof of Hardship or Crisis (Optional but Helpful)

  • Court Documents

  • Pay Stubs / Income Verification

  • Disconnection or Eviction Notices
